Classes for Individuals or Caregivers
These, Stanford University, evidence-based self-management classes are meant for individuals with chronic conditions or their caregivers. These will be held once a week for six weeks.
Available Classes
Diabetes 101
- This is a 2-hour introductory and overview diabetes education class that allows clients that are at risk of diabetes, pre-diabetics, diagnosed diabetics, caregivers, and those that need some diabetes education review or those that have questions. It is a free class made by appointment. It is taught by experienced program staff with diabetes training education background. There is a registered nurse who guides this education along with an education specialist.
Diabetes Self-Management Program (DSMP)
- The focus is to assist and educate clients on how they may be able to be better informed about diabetes and management of this condition to minimize and / or avoid complications that may occur from diabetes. There are two trained and credentialed individuals providing the class education. Some of the topics include:
- What is diabetes?
- Healthy food choices
- Healthy eating
- Physical activity
- Sick days
- Foot care
- Medication
- Depression and positive thinking
- Working with the healthcare system
- Goal setting and problem solving each week, and more
Chronic Disease Self-Management Program (CDSMP)
- This format is similar to the DSMP. It has a focus of chronic disease management.
