ASSIST OTHERS BY BECOMING A PATIENT NAVIGATOR
What is a patient navigator? A patient navigator is a member of the health care team who helps patients “navigate” the health care system and get timely care. Navigators help coordinate patient care, connect patients with resources, and help patients understand the health care system.
Patient navigators work in many areas of health care. Many of them focus on one chronic disease area, such as cancer, heart disease, or diabetes.
Who are the courses for?
The courses are meant for patient navigators, social workers, nurses, and community health care workers. Choose from two levels of training: level 1 for new patient navigators and level 2 for more experienced navigators.
New navigators learn strategies to address patient barriers, maintain professional relationships, and develop health promotion skills. More experienced navigators can focus on topics related to care coordination, emotional aspects of diseases, or legal and ethical health care issues.
